KVKK Application Form
Pursuant to Article 11 of the Personal Data Protection Law No. 6698 ("KVKK"), which was published and entered into force on April 6, 2016, certain rights have been granted to personal data subjects. You may submit your requests regarding your personal data processed by our company, Modalco, located at Karşıyaka mh. Odabaşı cd. Küme Evler No:4089/3 Mamak/Ankara in the capacity of data controller, by filling out this form completely and clearly, and signing it. Once the form is signed, you may submit it by hand with a wet signature to the address mentioned above, via secure electronic signature to our email address info@modalco.com, or by sending a scanned copy of the wet-signed version to the same email address from the email address registered in our system. We will respond to your application within at most 30 (thirty) days.
If the information and documents you provided to our company in your application are incomplete, or if your request requires additional costs beyond the scope provided, we will contact you and request your approval for the continuation of the procedures.
The right to apply to our company with this form, prepared in accordance with Article 11 of the KVKK, belongs directly to the personal data subject. For applications to be made on behalf of third parties, a power of attorney suitable for representing the relevant person must be present.
